Requirements

  • Completely filled up application.
  •  Valid passport 6 months from the date of travel.
  •  (2) recent colored Passport size photos with white background.
  •  Original & Photocopy of Biopage, valid visa/s and arrival stamps to OECD member countries for the past 5 years (If applicable).
  •  Bank Statement and Bank Certificate
  •  copy of ITR (Income Tax Return) or Form 2316
  •  Proof of residency
  •  Letter of Intent to be addressed to the consul
  •  Flight and Hotel Reservations
  •  Daily Itinerary for your travel
  •  Travel Insurance amounting to 2.5M

Employed

  •  Original Certificate of Employment
  •  Leave of Absence

Business Owner

  •  DTI and/or SEC Business Registration Certificate.
  •  Business Permit of Mayor’s Permit.

Student / Child

  • Original Affidavit of Support of Parents / Guardian
  •  School Certificate and ID
  •  Birth Certificate

Additional Remarks:

  •  If you are a senior citizen make sure to submit your Senior Citizen Card(Copy)
  •  If you are a housewife please submit letter of sponsorship and COE of husband, Marriage Certificate, Bank Certificate ans Statement of Husband.
  •  You could also submit your time deposit, Land Titles, Car OR/CR and other properties that could be use as leverage during your application
  •  Please do not leave any blank in the application form. Kindly put “N.A.” if it’s not applicable.
  •  Make sure that the back page of the passport is completely filled-up (Address, Tel Numbers and Name of Person to be contacted in case of emergency)

How to Apply

  1. Make sure to complete all requirements stated above.
  2. Bring all documents stated above in our office for checking.
  3. Pay at the office Visa Assistance Fee of P2,000 per person and Insurance for your travel.
  4. Attend your Visa Appointment to your corresponding embassy
