Visa Requirements
- Completely filled up application.
- Valid passport 6 months from the date of travel.
- (2) recent 2×2 size photos with white background.
- Bank Certificate (Must provide proof that you have enough money for your visit to US).
- Latest BIR-issued Income Tax Return (ITR) with BIR-received stamp and issued for the last taxable year (Original and Photocopy)
- Land Titles, Car Registration, Stocks etc.
- Invitation letter (From your Sponsor / Relative / Friend. If applicable).
- Marriage Certificate / Birth Certificate (PSA Authenticated).
- Employed
- Original Certificate of Employment
- DTI and/or SEC Business Registration Certificate.
- Business Permit of Mayor’s Permit.
- Original Affidavit of Support of Parents / Guardian
- School Certificate and ID

Additional Remarks
- If you are a retiree make sure to submit your Senior Citizen Card(Copy)
- If you are a housewife please submit letter of sponsorship and COE of husband, Marriage Certificate, Bank Certificate ans Statement of Husband.
- When Submitting COE Please include: Salary Details, Leave of absence & HR Email Address.
- Please do not leave any blank in the application form. Kindly put “N.A.” if it’s not applicable.
- Make sure that the back page of the passport is completely filled-up (Address, Tel Numbers and Name of Person to be contacted in case of emergency)
- Please submit complete documents to avoid delays in processing.
- Release date may depend on the embassy’s decision and may be prolonged if unavoidable circumstances would happen.
