Visa Requirements
- Completely filled up application. (A4 Size)
- Valid passport 6 months from the date of travel.
- (2) recent colored Passport size photos with white background.
- Original & Photocopy of valid visa/s and arrival stamps to OECD member countries for the past 5 years (If applicable).
- Original Bank Statement. (If E-Statement, you must ask your bank to certify it as true copy)
- Original Bank Certificate.
- Copy of Income Tax Return. BIR FORM 2316 / BIR FORM 1701
Added Requirements for the following:

– School Certificate and ID
– Birth Certificate when traveling with parents[/tg_accordion]

Additional Remark:
- When Submitting COE Please include: Position, Date Hired, Compensation, Office Address, HR Landline Number (Cellphone Number is not allowed) & HR Email Address.
- Must be printed in A4 and filled up by computer.
- Please do not leave any blank in the application form. Kindly put “N.A.” if it’s not applicable.
- Make sure that the back page of the passport is completely filled-up (Address, Tel Numbers and Name of Person to be contacted in case of emergency)
