Visa Requirements
- Completely filled up application. (A4 Size)
- Valid passport 6 months from the date of travel.
- (2) recent colored 4.5 cm x 4.5 cm size photos with white background.
- Original NSO Birth Certificate issued within the year
- Original NSO marriage certificate issued within the year
- Original bank certificate.
- Copy of Income Tax Return. BIR FORM 2316 / BIR FORM 1701
Added Requirements for the following:

– School Certificate and ID
– Birth Certificate when traveling with parents[/tg_accordion]
[tg_accordion title=”Housewife” icon=”” close=”1″] Submit letter of sponsorship and COE of husband, Marriage Certificate, Bank Certificate ans Statement of Husband.[/tg_accordion]

Additional Remarks:
- If you are late registered in your birth certificate you are required to submit an Original Baptismal Certificate and Form 137
- Please print / Xerox all the documents to A4 size.
- No staple allowed (To all requirements).
- Please do not leave any blank in the application form. Kindly put “N.A.” if it’s not applicable.
- Make sure that the back page of the passport is completely filled-up (Address, Tel Numbers and Name of Person to be contacted in case of emergency)
- Hotel and Flight Reservations is not required.
